What Was The Incident?

At a waste management company site operatives were processing powder fire extinguishers that had reached the end of their serviceable life. While discharging a cartridge type extinguisher there was a sudden release of pressure and the unit exploded, striking a worker in the head and embedding the extinguisher in the ceiling of the workroom. The worker received emergency treatment but died the following day. The HSE found the employer had not produced a suitable and sufficient risk assessment, their systems of work for processing these extinguishers were not suitable, old extinguishers were not stored appropriately prior to processing, and there was no restraint or containment system to prevent dangerous parts being ejected during discharge.

What Was The Outcome?

The employer pleaded guilty to breaching Section 2(1) of the Health and Safety at Work etc. Act 1974. The company was fined £40,000 and ordered to pay £17,034.25 in costs.
